INFORMATION TEXT WITHIN THE SCOPE OF THE PERSONAL DATA PROTECTION LAW NO. 6698

1-PROTECTION OF PERSONAL DATA:

As Trakyadent Çerkezköy Oral and Dental Health Center (Pandar Dentistry Private Health Services Industry and Trade Co. Ltd.) and Trakyadent Branches, the processing and protection of personal data within the scope of the Personal Data Protection Law No. 6698 are among our top priorities, and we make every effort to comply with all applicable legislation. As the Trakyadent brand, in this context and in line with the principles determined by the legislation, we will take the necessary measures to ensure the lawful protection and processing of personal data and will also organize the required internal awareness for our employees and business partners on this matter.

2-COLLECTION AND PROCESSING OF PERSONAL DATA:

Your personal data may vary depending on the health services provided by our Oral and Dental Health Center and branches. Depending on the service provided, they may be collected verbally, in writing, or electronically through various methods (documents and forms issued for you and submitted to your approval, call center, website, social media platforms, mobile applications, and similar tools). In this context, all personal health data required for the execution of examinations, medical diagnosis, treatment, and other services, as well as other personal data and special category personal data, may be obtained as listed below:

Your name, surname, Turkish ID number, passport number or temporary ID number if you are not a Turkish citizen, place and date of birth, marital status, gender information, and the photocopy of your Turkish ID card or driver’s license you provide,
Your contact information such as address, phone number, and e-mail address,
Your financial information such as bank account number and IBAN number,
Your medical data such as laboratory and imaging results, test results, examination information, and prescription details obtained during the execution of medical diagnosis, treatment, and care services,
Your responses and comments regarding the services you received from our center and branches,
Your CCTV recordings taken during your visit to our center and branches,
Your voice call recordings taken when you contact our Call Center,
Your private health insurance information and Social Security Institution data, if applicable,
Browsing data obtained during the use of our website, IP address, browser information, and any medical documents, surveys, form information, and location data you voluntarily provide.

3-PURPOSES OF PERSONAL DATA COLLECTION AND PROCESSING:

Your personal data are collected verbally, in writing, or electronically to ensure the provision of our services in line with the purposes stated above, and so that our Center and branches can fully and properly fulfill their responsibilities. The personal data collected through these methods may be collected, processed, and transferred within the scope of the conditions and purposes specified in the Law, in compliance with the fundamental principles set out by the Personal Data Protection Law.

Your personal data may be processed by our Center for the following purposes within the scope of the rules and personal data processing conditions and purposes determined by the Law:

Protection of public health, preventive medicine, execution of medical diagnosis, treatment, and care services,
Sharing requested information with the Ministry of Health and other public institutions and organizations in accordance with the relevant legislation,
Informing you about your appointment through our Call Center and Digital Channels,
Verification and identification of your identity by our healthcare workers and Call Center departments,
Planning and managing internal operations by the management of our center and branches,
Conducting analyses to improve the quality of health services provided by our center and branches,
Issuing invoices for the services you receive by our accounting and related departments,
Resolving any problems related to the health services you receive from our center and branches and responding to your complaints,
Taking all necessary technical and administrative measures within the scope of data security by our center and branches,
Planning and executing the human resources operations of our center and branches,
Ensuring technical and commercial–operational security of our center and branches,

The “Personal Data” mentioned above may be stored with great care and diligence by us and other service providers with whom we have a business relationship, in accordance with the provisions of the legislation.

As the Trakyadent brand, our center and branches approach the issue of personal data security with utmost diligence and attention, and with this awareness, process the personal data of data subjects within the limits explained below and determined by the legislation. Your personal data are stored for the period required for the purpose for which they are processed and for the duration prescribed by the applicable legislation for the relevant activity. At the end of the specified retention periods or upon request of the data subject, personal data are destroyed using designated destruction methods (deletion, destruction, or anonymization, etc.).

4-TRANSFER OF PROCESSED PERSONAL DATA TO THIRD PARTIES AND INSTITUTIONS:

Your personal data collected by our center and branches may be transferred to the following third parties and institutions within the framework of the Personal Data Protection Law No. 6698 and relevant legislation, limited to the purposes stated in this document, and in accordance with Articles 5 and 6 (processing conditions) and Articles 8 and 9 (transfer conditions) of the Law:

The Ministry of Health and its affiliated units,
Private insurance companies (health, pension, life insurance, etc.),
Social Security Institution,
Judicial authorities,
General Directorate of Security and other law enforcement agencies,
Chamber of Dentists,
Laboratories, medical centers, ambulance services, medical device providers, and healthcare providers with whom we cooperate for medical diagnosis and treatment,
The healthcare institution to which the patient is referred or applies to,
Your attorneys holding a Special Power of Attorney,
Your legal representatives,
Other service providers and consultants we work with, limited to their duties,
Regulatory and supervisory authorities and official bodies,
Other institutions required by legislation,

5- YOUR RIGHTS UNDER ARTICLE 11 OF THE PERSONAL DATA PROTECTION LAW :

The rights granted to individuals whose personal data are processed under Article 11 of the Law are as follows:

To learn whether personal data are processed,
If personal data have been processed, to request information regarding such processing,
To learn the purpose of personal data processing and whether they are used in accordance with this purpose,
To know the third parties to whom personal data are transferred domestically or abroad,
To request correction of personal data if they are incomplete or incorrectly processed and to request notification of such correction to third parties to whom the data have been transferred,
To request deletion or destruction of personal data in the event that the reasons requiring their processing cease to exist, even if they have been processed in accordance with the Law and relevant legislation, and to request notification of such destruction to third parties to whom the data have been transferred,
To object to results against the individual that arise from analysis of personal data exclusively through automated systems,
To request compensation for damages in the event of unlawful processing of personal data.

To exercise the rights listed above, you may submit your request specifying the right you wish to use by completing the Data Subject Application Form available on www.trakyadent.com.tr
. You may deliver a signed hard copy of the form along with identity verification documents in person to “G.MKP. Mahallesi Atatürk Cad. Mehtap Sk. No 7 Emre Pandar İş Merkezi Çerkezköy/Tekirdağ” or to the relevant branch address, or you may send it through a notary public or via other methods specified in the Law.

Your application will be finalized free of charge as soon as possible and within no later than thirty (30) days, depending on the nature of your request. Applications must be made in Turkish. Applications must include the applicant’s name, surname, signature (if in writing), Turkish ID number for Turkish citizens, nationality and passport/ID number for foreigners, residential or workplace address for notifications, and e-mail address, telephone, or fax number if provided, as well as the subject of the request.

It is not possible for third parties to make a request on behalf of personal data owners. For a person other than the data owner to submit a request, a special power of attorney issued by the data owner specifically for this purpose must be provided.

6-OTHER PROVISIONS:

No statement in this Information Text may be interpreted as limiting any rights guaranteed by the Personal Data Protection Law. This Information Text is subject to Turkish Law and shall be interpreted accordingly. We may update this Information Text occasionally by publishing the current version on our website.
